 * HI, after answered all the question in questinaire and clicking submit response,
   i am receiving 404 error which poting index.php. DID i did some mistake ?

 * Sorry to hear about your trouble. The most likely cause is some other plugin (
   or your theme) preventing the javascript for Awesome Surveys from loading or 
   working properly. There is a known issue that may be causing your 404 error [http://wordpress.org/support/topic/404-error-when-submitting](http://wordpress.org/support/topic/404-error-when-submitting)
   Have a look at that and if that doesn’t fix the issue for you try deactivating
   other plugins and switching to the Twenty Fourteen theme.
