 * Is it possible to eliminate the user requirement to submit name and email? For
   entertainment value, simply take the quiz and be routed to the page based on 
   score.

   having to enter their name and email address. Is this possible?

 * These options are already there, uncheck Name and Email under WordPress Simple
   Survey Settings
