That’s the problem.
If the plugin is activated, the CSS for it should be loaded on the page and it is not. This indicates that either the plugin is not active or that the CSS has been disabled (did you disable the CSS?)

Yes, I disabled the CSS.
As I said the plugin is already active. The problem is only in some post, for example the one I linked; in other posts it works well.

On the page where you showed [ez-toc] can you please scroll down and take a screenshot of Table of Contents settings section?
The minimum number of heading is set to 4 and only H2 are to be used as TOC item.
On this page I only count 3 H2 headings:

Try changing the the heading count to a lower value.
