 * Tested and the issue is with the theme no plugin, I placed 2 orders by activated
   default theme and this is working. does this mean I need to change my theme completely?

 * That will depend on the theme and what support is available for it. You may want
   to reach out to the theme developer to see if they can help with the incompatibility.
 * If not, you may need to switch themes to one that supports this extension.
