This may be because plugins conflict like in this thread: https://ww.wp.xz.cn/support/topic/bookly-plugin-conflict/
Some third-party plugins override the default jQuery.fn.tooltip function with another one. WordPress has a built-in tooltip library jquery-ui-tooltip (see Core-Registered Scripts). Conflict may occur if third-party plugins override built-in libraries.
Try to disable the conflicting plugin on the Form Edit page. Do a conflict test to detect the conflicting plugin. See How to do a plugin/theme conflict test?

I don’t think it’s a plugin conflict. I’ve tried installing older version of UM and it’s worker fine. I only experience the problem on the latest versions.
I have same problem. I have tried to install 2.6.5 version, it’s work!

Please use the latest version of the UM. Also, please try to do a conflict test just for testing.
