Upload problem, need help

Hello!

sometimes i have problem with image uploading.
I choose image, click upload button and in a second after, upload part of wordpress admin become blank, but it should show last file that i had uploaded.
Do you know how to fix it?
P.S. btw everything is ok with uploading, file is on server.

Help please
