 * I am a novice user of WordPress and PHP at best. Do I have any shot at all of
   creating a form that populates a SQL database.

 * What do you want specifically.
    There are countless form plugins for wordpress,
   but if you just want the data to go into MySQL in a specific manner, it would
   help if you told us how. I assume that you know how to create the form. Writing
   the processing code is easy.
