 * Hello;
   Thanks for this great job. I have a question. How can I customize the 
   appearance of the fields we add to the form? Each field is added one after the
   other. How can I make the fields appear side by side (I want them to be side 
   by side like the name-surname field)?

 * You can make your fields side by side or whatever width you want using “Field
   Size (width)” option.
