visual editor and insert image button broken

    I’ve been adding some posts, and suddenly I’m only able to edit and add new posts in HTML format only. The insert image link is broken in Firefox and Chrome. As far as I know, I have made no changes to WP core files.

    In IE, I am able to see more details on the error. Anyone seen this before?
