The form code uses an iframe to display it. Also, the form itself is very tall and I’d like to reduce its size by 25-50% if possible. I thought I had set the document permissions for anyone who had the link to be able access the screenshot image. * At any rate, I discovered that the settings for the slideshow displaying above the form in my sidebar were causing the blank space. I changed the slideshow format and now everything displays fine.
