 * so i am new to this forum , please correct me if i am wrong in any way.
    I am
   looking to build a form that wil be added first to my page of the site link i
   mentioned above. the form i want is something like this

 * their type of form is quite compelling i am looking for something like this to
   work with, so if anyone can tell me plugin they are using or stuff like that 
   i would greatly appreciate it !
