I have the same issue with this plugin update, 4.1.0
steps to reproduce:
login using wp-admin, and 2FA using Authenticator app and then you will be redirected to /wp-login.php?action=validate_2fa page with below error message.

But If I go to wp-admin after I login, I can access my dashboard.
