I am facing exactly the same error, kindly help. whether its Yoast premium or Yoast free version I am facing same issue. Even I checked in the health check, everything seems to be healthy

Here is some more details on this. Looks like my site admin id and password was brute forced and then a module was installed which helped inject the malicious codes.

I have been seeing this thing getting added in the headers in my website too, My wordfence alerted that some one was logging in from an admin account and adding those lines. I removed all admin accounts and changed the password of my admin account, This has solved the problem for now.
Take these steps immediately:
1) Install wordfence
2) Scan the entire site
3) Remove all admin accounts except yours
4) change your password
5) change your username from default admin to something randomHope this helps, will update here if the problem re-occurs
