I have confirmed the file keeps getting deleted from my server NOT deactivated.
I have added the file from scratch several times and something keeps deleting it.

No, didn’t change the file name. I have done reinstall again and the plugin is still there currently. It is active. But I am scared to update it because I am tired of reinstalling.
