This is actually overly-zealous logging. What you’re seeing here is totally normal and expected based on how some gateways use fragments.
I’ve turned off that logging and this will be released in 10.3.4. Thanks for the nudge on this!

WooCommerce changed how the “Hide shipping costs until address is entered” option worked in 9.8.2+ (this was most likely an unintentional change)
We had to make some adjustments to ensure that shipping did not appear to be “Free!” when it was not free.
Sounds like we may need to further adjust this to account for your situation. Can you send a screenshot of these settings:
Default customer location (WooCommerce > Settings > General)
Shipping zone configuration (WooCommerce > Settings > Shipping)
Shipping settings (WooCommerce > Settings > Shipping > Shipping settings)Thanks!

There was a mistake in 10.1.10 that seems to have affected a small number of customers on older PHP versions.
We have updated 10.1.10 to fix this. You can redownload it and install it in place of the current version:

Good question – I don’t remember exactly why that hook isn’t called. Strangely in 5 years you’re the first person to ask about it, so it seems like a hook that is pretty rarely used.
To avoid potentially complicating things, I would recommend using this hook instead:
cfw_after_cart_htmlHowever, please note this hook doesn’t fire within an HTML table so you may need to adjust your output a bit.
