I am having I think the same issue. everything else seems good, but when I get to the form itself, I cannot select a time to book an appointment.
I don’t think I can give you a link because its in draft status on my WP site.
I have 1 location, 1 provider, and 1 service. They all show up, but you cannot choose a day or time. What setting did I miss?

I had the same issue, but the above solutions “save changes” in the settings tab resolved mine as well. Perhaps a note to remind newer WP installers to save before trying to view?
