I am also having this problem. It appears that the path is adding the blog directory twice. In my case, my blog is in a folder called blog (how original). Instead of the image using:
/blog/wp-admin/images/media-button-image.gif
It uses this:
/blog/blog/wp-admin/images/media-button-image.gif
