After trying another form plugin, I discovered it was the theme that was at fault here. Luckily their latest update has sorted it out.
However, both my client and I prefer the other’s appearance. Thank you for your help anyway. I’ll mark this as resolved.
Cleared cache, still the same.
Just upgraded WP from 4.2.2 to 4.5, but it looks the same in both.

I am still seeing only dots, no boxes, and when I click on the dots (just in case) nothing happens.

As I said in my post, it is happening BOTH on checkboxes and on radio buttons. I tried both options and both gave the SAME result.
