My problems with Send PDF for Contact Form 7 started when dreamhost updated their unix system. The first problem was a never ending spinning arrow after clicking the send button, I figured this would be a problem with the server that would most likely be cleared by morning; and it was. The second problem was the fact that the Send PDF for Contact Form 7 was gone from the plugins folder on my wordpress site, trying to reinstall the plugin gave a notice saying that the missing plugin folder did exist so, I started FileZilla and had a look on the server and sure enough the folder was there but, with rather less files than it should have had. Fortunately I had used FileZilla to do a full backup of the website to my home computer just a few weeks before hand so was able to reinstall the missing files which, finally made Send PDF for Contact Form 7 visible and updatable on WordPress again.
Once I had the plugin fully installed again I had to redo the settings on the plugin’s General Settings page and redo the code at the bottom of the General Settings page for outputting my form to a PDF.

If I hadn’t had a backup of the website I would have use FileZilla to delete the Send PDF for Contact Form 7 folder from the server then, gone to logged into WordPress and reinstalled the plugin a fresh.

I suspect that the Dreamhost server update has damaged a number of links to files on my website that WordPress will no longer see and so will not be displayed in the media window/folder.
