By what I read, you need to create an Employee to put an appointment form on a page. Is that correct? I am assuming only the paid version allows you to have customers create an appointment.

I still have the sidebar and admin header but the page content is blank.
I’m running PHP 7.0.31 and had deactivated all plugins.
I set up debugging in a site which did have other plugins active. Results as follows.
