In 2.9.1 we had this problem, and went into the wp-members-2012 style sheet and edited at about line 157.
there was a top padding of 16px that we changed to 0, and the field titles lined right up with the actual fields on the right.

We need to require a phone number for contact, but it seems “XU” will satisfy as a phone number.
I am interested in the same thing I think. We offer monthly services, but they require a setup fee. One product is the montly subscription, and the other is the one time setup fee. We need help figuring this out.
