Update Gallery to version 4.0.3 and fresh install of Lightbox 3.0.0 solved the problem.
There is still the same problem. I have updated from wordpress admin panel. Lightbox is 3.0.0, Gallery is 4.0.1 and still fatal error occurs.

On my site it was the same. Update to 3.0.0 crashed site. I had to downgrade to old version to restore functionality.
