Lucian has successfully identified a problem with some missing php extensions. In my case, on a web server running FreeBSD 14.3 with apache24 and php83, the following additional packages had to be installed:
- php83-xmlwriter
- php83-ctype

sorry for the late reply. I can confirm that I have tried a clean install of the plugin, essentially installing it immediatelly after deleting WP files, dropping the DB and then recreating the WP site from scratch. Same result.
My hosting environment is a FreeBSD VM with apache24, php83 and a bunch of php83 extensions. The exact packages used are listed below:

Below are my answers to your questions:
- When I reported the problem, there was only one user on the system. For testing purposes I created an unprivileged user. I am having the exact same issue with that as well – if I click on “Configure 2FA”, it just jumps to the top of the profile page. I have also noticed that with both users the “2FA Status” shows as “User has not logged in yet, 2FA status is unknown”, despite both users logging in and out several times.
- This is my brand-new site which I am using to learn WP. This morning I tore it down and rebuilt from scratch, with WP-2FA being the only plugin activated (besides the default akismet and hello-dolly which are both de-activated).
- The site is running the default 2025 theme.
Hi @lucianwpwhite , thank you for looking into this. I have fired up the developer console in both Firefox and Edge. There is not much happening in either of them once I click on “Configure 2FA”.
