Okay, so after much playing around with it, it seems that the issues are coming from posts that have some kind of formatting in the last line (h6, ul, etc) and where there is no after that final paragraph. If I add the break, the ad goes where it should. If the formatted text is the last line of the post, the ad inserts in the paragraph before it.
Not sure if this is a bug (you might still want to take a look at it), but at least now I have the answers to all my questions! 🙂

Just went through the support threads and figured out that question 1 was due to caching, so that’s resolved.
The “after paragraph” issue is still a mystery though!
