 * Hello Marko,
 * I have resolved the issue! There was some deprecated code in my functions.php
   file that was causing jQuery to defer and thus load after any javascript was 
   run. I have removed the deprecated code, and the setup guide is now working.

 * The website is hosted using InMotion Hosting.
 * I did try skipping the setup guide using the mentioned link, but that also did
   not work.
 * I found many errors on the page related to the setup guide when I inspected it
   as you suggested. I am not certain what needs to be done to resolve them. Here
   is a screenshot of what I discovered:

 * After reaching out to the Enfold team, they provided this solution, which worked!
 * Please try the following in Quick CSS under Enfold->General Styling:
 *     ```
       form.forminator-custom-form {
         display: block !important;
       }
       ```
