Okay, I am an idiot. The tech support at my hosting service, Laughing Squid, gently pointed out that I installed WordPress in a nonstandard configuration. Specifically, I have WordPress in its own WordPress directory, not the root directory.
When I updated the index.php file, it began pointing to where wp-blog-header.php would be in the default directory location, not where it actually was in my filesystem. Problem solved.

I just installed WordPress 2.0.4, and I’m having the same problem as fivesilver. The update from 2.0.3 to 2.0.4 seemed to go well, dashboard looks fine, and I’ve removed all the non-standard plugins, but I get a blank page when I try to view the site.
