Nevermind, I think I’ve got it figured out. I accidentally clicked a page builder button that I think conflicted with the current theme on this page and that broke it. I’ll mark as resolved.
Thanks for the reply. Please note, I’ve turned off the submission lifespan limit and made it date-based instead. The form is time-sensitive and I’m just keeping a manual eye on submissions. No submissions have been deleted and the total remains below 50 for the whole lifespan. Here is the form export.
