It goes away if I remove the title=”0″ bit.
Same for my live and dev sites.
Any thoughts?

This page is already built-out and provides a place for them to submit their username and receive their credentials via email–all part of a separate system through OfficeAutoPilot. Redirecting to that page from the “forgot password” link would be just as helpful.
