The link for my form is https://eternalspringfilm.com/
If you open this link on a Windows browser, the form will be in the second section right after the banner. But if you open it on an iOS browser, the form section will be empty.

Thanks for replying.

My iOS version is iOS 15.6 (up-to-date version). Browsers are system-installed Safari and Google Chrome 103.0.5060.63 version.
The problem only occurs on my iOS phone and macOS laptop. My Android phone and Windows desktop do not have this problem.
