When you view the page using the address above, all is good and the fields are present. It was permalink settings that caused the trailing slash to disappear. * That has been sorted… unfortunately, the page isn’t populating with the password reset form. The “problem” was caused by Google Chrome assuming I was in the US and formatting the date accordingly.
