I’m running the check on my staging sites as I always have and I always used WP 2FA – Two-factor authentication for WordPress plugin.

Try the usual troubleshooting process:
Disable all plugins, switch theme and test.

I sent the debug information. I noticed your plugin states MySQL Version: 5.5.30 while WordPress site health shows Server version 5.7.18-15-57-log
There is a proxy on my managed hosting server so makes it a little confusing to know which DB it’s getting this info from.
I have my hosting may be to blame for these strange issues.

I have it setup to enforce ‘All users’.
I don’t have any custom dashboard or login URL either. Waiting for my host to update mysql as it’s quite out of date(mysqli 5.7.18) and hoping maybe that will fix it.
Even if I log in with the new account I still see “this user has not logged in yet, 2FA status is unknown”
Could it be that I am logging in from the same device with a new account?

Thanks for getting back. I will use the email address for the premium questions I have.
As for the deactivating, it’s actually for cases where I can’t log into the dashboard for whatever reason, then my only option is disable the plugin in SFTP. Could be for troubleshooting purposes as well. I was blocked a few times from logging in due to 2FA plugin not working correctly so that’s my main concern but it’s not the end of the world.

Thanks for the suggestion as well regarding the table cells and you are correct, I can reproduce the bug when deleting all the rows with header present, you can no longer add them back.

It’s polyalng redirecting, even in the response header it states x-redirected-by: Polylang.
Also as soon as I disable Polylang it works 2022/?cat=843.
So I rollback to find which version this began in and found it started at 2.9
Now in the changelog the most suspect change is with the taxonmoy terms:

* Fix canonical redirect for taxonomy terms
I’ll open another support thread as this sounds like different issue.

Ran into same issue in category archive when using string query parameter with date in (ex. en/2022/?cat=843) Polylang redirects(301) to the category page but shows all years instead of just 2022 and query string paramater disappears.
Anyone fixed this or still running on old polylang version?

Good to know about the plugin for troubleshooting although I already did this troubleshooting manually:
I tried switching themes, deactivating all plugins, resetting permalinks, repaired the DB with WP auto repair but still no pagination was present.
Anything else that could be causing this?
Sounds like browser stack caught a legit bug but I doubt @takayukister will fix it in the support threads, you can go to his CF7 github and create a issue instead and maybe get more attention there.
np..How old are those versions and what are the usage stats?
