This is quite unexpected, I admit. One of these buttons looks like it’s either not coming from Formiantor or… it’s actually the same button but some JS and/or CSS is failing so it’s visible (Forminator uses some JS/CSS to make buttons look and work as they do).
I’d like to take a closer look at the form and test it then. Could you export the form and share export code with use?

Thank you for the export! I’ve been able to replicate this issue on my test site with a default theme as well. Looks like it may be a bug in this case.
I’ve also found a solution to fix this. Please enable the Custom CSS option at the bottom of the Appearance tab of the form and add the following CSS:
#forminator-field-post-image-postdata-1 {
display: none !important;
}

As it is a recent issue we can’t give an estimated time to release a fixed version but Forminato receive updates frequently and I hope we can fix it as soon as possible,
