    How do you want to link the document? Since this question is posted in the WordPress Google Support forum I am assuming you want to integrate the Google Form with your web site.

    You’re getting a page not found error which means the page you have added to the menu for your site no longer exists. Before adding the form to the menu, I would first make sure that the form works within the web site.

    Did you define a Google Form within WordPress using the URL you posted above? If not, that is the first thing you need to do. It is simple to do that, simply choose the “Add Google Form” from the Dashboard menu.

    Once you’ve added the form, there will be a “View Google Form” button near the title within the editor screen. Clicking on that will let you view the form within the context of your site.

    Because WordPress Google Forms defines forms as custom post types, they can be added directly to your menu using the standard menu functions – your forms will appear as options to add to the menu just like pages, posts, links, and other items do.

    Hi Mike – thanks for your response.

    Yes – I would like to integrate the form into the site. My friend did it well here: linkandg.com (RSVP)

    I can go directly to the form on my browser, and if I use the “view google form” button on the editor screen it shows the form. I added it using the “add google form” from the dashboard.

    I really appreciate the help.
