Update – I deactivated the plugin, but it is now definitely broken – if I try to activate it again, I get a “plugin activated” message, but the link under the “active” plugin in the list of plugins still says “activate” rather than “deactivate.”

Just checked; AO seems inactive and your site renders normally, so I guess you succeeded in fixing this.

Regarding an autoptimized JS-file not being found; this typically happens when you’re changing AO settings and clicking on “save changes & empty cache” while also having a page cache (cloudflare + maybe other) which holds HTML in cache which still references the old JS. The fact that changing AO options did not change a thing confirms this assumption. In that case you should either click “save changes” (but not empty cache) in AO or also empty your page cache.

The active/ not active issue is something I (or AO) does not control I’m afraid, plugins being active or not is core WordPress functionality.
