What URL does your browser show when you get these 404 pages?
It comes up as 12.34.56.78/404.html. When I deactivate the plugin, it is 12.34.56.78/~mysitename.
So is your WordPress site in a subfolder?
Or are you using a Multisite setup?
It’s a bit of a weird situation in that, until we go live, we are still hosting the site on the first developer’s server. We are no longer using this developer, and there has been another person in between the developer and myself working on the site. From what I understand, the first developer has quite a few other in development sites on her server. Does that help?
It’s not possible to tell from that.
If you urgently need to password protect your development site you could try installing the Members plugin which has a setting that only logged in users can see the site. That would require you to create users for each person you wanted to view the site who would need to login with username and password rather than just password.

While using it, its the same issue I am facing but in multisite wordpress site. My domain is third level domain where this wordpress is intalled where I am using it e.g. (wp2).wp1.com It does show the enter password screen but once we enter the p/w it keeps waiting and give 404 error after a min. If I remove the activated password site works fine. What is the alternative to correct his issue? Thanks.

I never ended up resolving the issue with this plugin. Instead I used the plugin Restricted Site Access.
