Again, thank you so much for your response. I had to respond here as gmail is giving me the following error when I try to reply to your response earlier:

    I reinstalled WP7, no change
I deactivated all plug-ins to see if there was an issue with another plug-in, no change
I installed WP Debugging plug-in and no errors popped up

I am at a loss and this is a good plug-in for what the department needs for the State Police Alumni.

Any ideas would be welcome. If you need it, I can even provide you access to the site to see.

    Thanks again in advance.
    Jim

Found the issue. The plug-in was being tagged with MOD-SECURITY rule 942360. Once I whitleisted that rule for the domain, it now works.

ModSecurity rule 942360 is part of the OWASP Core Rule Set (CRS), specifically located in the REQUEST-942-APPLICATION-ATTACK-SQLI.conf file. Its main purpose is to detect concatenated basic SQL injection (SQLi) and SQL Local File Inclusion (SQL-LFI) attempts
