404 on submit

I’ve been using your plugin for donations for quite a while but after recent updates to WordPress and plugins, WP Payeezy Pay is no longer working. When I hit the Donate Now button, I get a 404 error (regardless of the active theme). I tested with the same plugins and theme on my local machine and it works as it should, redirecting to the secure First Data Payeezy page. Any idea what the issue could be? Here’s the URL:
