404 Page Not found error – problems

I have been searching around these forums, and have tried several things but have not been successful. It first started where I was getting the 404 error only when I navigated away from my home / main page and then tried to go back to it. After I did this and reset my permalinks to “default” it is now doing it with all of my pages… any suggestions?
