are you using Custom Fields extension or just the forms API?

With custom fields extension you can add the options manually one by one or register data source (if you have some PHP programming experience), see “Using Registered Data Sources” here https://wpadverts.com/documentation/custom-fields-paid-addon/

If you are using Forms API then you can add options the dropdown as explained here https://wpadverts.com/documentation/forms-api/
