500 Error When Authenticating by Email

    Just recently ran into an issue where clicking the “Accept Transaction” link in the One Time Passcode email throws a 500 error as seen here http://pho.to/AkGNo. Can’t login to any website that has the plugin enabled unless I disable it through FTP.
