To add the form to the sidebar (or any other widgetized area), simple use a Text Widget and use the shortcode [optinform] as the widget content.
This will output your optin form.

If I add this to my sidebar, can I also have a different sized optin on my posts (2 different sizes, wide for posts and narrow for sidebar)?
Yes, but you will need some custom CSS to accomplish this. I took a look at your website, but I can’t find a page where there is an optin form added.
