Accessibility – aria

Hi, The form at the bottom needs aria label fields. How do I add properly to the following:

[text* Name placeholder “Name”]

[tel* Phone placeholder “Phone”]

[email* Email placeholder “Email”]

[textarea* Case placeholder “Describe Your Case”]

[submit “Submit”]
