Activating App Id

Facebook is now requiring apps to be submitted and reviewed. What are the items and descriptions I need to include in the submission?

(I created an app, and it did work for about 24 hours, but now the FP App dashboard shows: “This app is in development mode and can only be used by app admins, developers and testers”, and the widget on the website is completely blank.)
