I guess this version is not compatible with the current version of BadgeOS/BuddyPress/bbPress?
I turned off all of my plugins, except for the ones related to this add-on.. and tried installing again. Still fails, but at least now I can see some errors.

Apologies for the delayed response as we have been busy updating the plugins and launching our new website.
We have double-checked the add-on with latest versions of BadgeOS (1.4.11) and BuddyPress (4.2.0) plugins and found the add-on working fine. Please elaborate the issue a bit more.
- Version of WordPress and PHP on your server?
- What are the steps to re-produce the issue?
- Which WordPress theme are you using on your site?
