Now I dont know where to go to access the plugin. I do not have a membership menu on the dashboard.

Have installed plugin successfully or any error occurred during installation?

Where do I create the categories?

You can create categories from Admin -> Posts -> Categories menu. You can use these categories in access level by dragging categories section from main rules in to edit access level section from Admin -> Membership -> Access Levels -> Edit level Rfer these screenshots http://ww.wp.xz.cn/plugins/membership/screenshots/
