Would you please edit the option and go to the Visibility tab. ake sure Widgets are selected for “Apply on” section.

If this option is already enabled, please let us know if you have any other Visibility rules added.

I can see the form on the homepage now, so I guess you’ve already added the homepage in the Visibility Rules.
Also, keep an eye in the all/any conditional 😉
