    Hello,

    I have checked everywhere and I cant seem to find where to added a title to the form.

    Any advice?

    Hi @vic1129,

    There is not an option to do this within the shortcode itself, however you could add the title text above it manually. For example like this https://i.imgur.com/K7eRFwP.png

    If you’re using the new Block editor in WordPress >5, you could use a Heading Block above the shortcode.

    Let me know if that works for you or if you have a specific requirement for a title in the shortcode.
