Adding Event: Most Options have disappeared

Hi,
When I try to add a new event, most of the options have disappeared.

Whereas the event results, box score, excerpt, user, etc should be showing, it doesnt.

Also, it only has one section to enter the home team, then nothing else is below it.

Why is this and how can it be fixed?
