Hi,
To be sure that you created the field, in the “Fields” page, can you see your new field on the right?
After you have created a new field, if you had the form edit page already opened, you have to click on “Save settings” (if you don’t want to lose the modifications) and reload the page to see on the left the fields created after you opened the form edit page.

No I cannot see the field on the right side. I’m choosing add a new field, field label name Preferred method of contact. Field type multiple check boxes, default value(s) Email|Phone. I choose add new field and nothing gets saved. Thank you.
Maybe you are saving the field but you got an error message, like a wrong character in the slug field.
I just noticed that the error message in this case is not visible very well, so check after you save if there is an error message under the title of the page.
I’ll make this message more visible in the next version, meanwhile please let me know if it solves your issue.
