Adding Metadata to Individual Posts

I know there are likely many previous posts for this question but I have not found the answer (not using the correct terminology?)

The over all need is to make sure that when posts are shared/liked via Social Media channels that I can define the Page Description and Image that is shared.

ex.: When this post is shared/linked by readers to Facebook the Page Description matches the default site description, not the post description.

I am looking for a plugin (or any hack) that will allow me to add unique META for title, description, keywords to individual posts.

I thought is could be done in Jetpack (have not found where) and in a Header Footer plugin (did no the ability to add metadata to individual posts).
